Newborn Model Call Application

Please fill out the form completely.  Incomplete forms will not be accepted.

The purpose of this form is to determine if you and your child(ren) would be suitable participants for the current creative project I'm working on.  Please be as thorough in your descriptions as possible.

Also, if you haven't already done so, please be sure to email in a picture of whomever might be participating (yourself, partner, siblings, etc.) 

Please tell me all about your pregnancy.  What is your due date?  Do you know if it's a boy or a girl?  Do you have a name chosen yet?  Will you be having a schedule c-section?

Please tell me all about your family.  Do you have other children?  What are their genders, names and ages?  What is their personality like?  Do you have any pets?

If I were to want to include family poses, do you already have clothing that would photograph beautifully?  Please describe here.  (I may also request that you send pictures of proposed outfits.)

The session will take place on a weekday morning.  Will that be possible with your family's schedule?  Please elaborate details regarding your availability.

Have you visited my website?  Is there any specific photo/style that you are drawn to?

Have you been professionally photographed before?  By whom?  Describe your experience.

Anything else you'd like to share?

By submitting this form it indicates that you understand that in order to participate in a creative session, you must sign a model release which grants the photographer the right to use and publish photos and video created during the session for any advertising, display, publication, marketing, competition and/or other studio purposes.
